Nurse reminders for the 2023-2024 school year for grades 6-12. Students who are age 11 or 16 or will be turning 11 or 16 this summer need to get the following state required immunizations to attend public school prior to the beginning of the 2021-2022 school year.  *Age 11 needs Tdap and first Meningitis (MCV4)  *Age 16 needs second Meningitis (MCV4)   If your child has an Exemption from immunizations I need a copy. Please read and sign the attached Glucagon permission form as well if your child will have Glucagon at school.  If you have an Epipen or inhaler I will need a plan of care from the physician on the first day of school. If you plan on your child self carrying an Epipen and or inhaler please have the doctor sign the attached form. Also, you will need to sign the attached form giving us permission to allow the student to self carry. If your child will be taking any medication at school you will need to fill out the attached medication form and bring it to me in the original prescription bottle with the current date. If you child has a seizure disorder, please have the doctor complete the attached Seizure Action Plan.
